CSA offers Flu Shot Clinic
Community Support Advocates is partnering with the Polk County Health Department to provide a Flu Shot Clinic on Tuesday, October 18, 2016 from noon to 2pm at the CSA office.
Most major insurance is accepted. Please bring your insurance card to the clinic. The cost for a flu shot for uninsured is $20. No one will be turned away for an inability to pay. A free light lunch will be offered to those who receive a shot. The Flu Shot Clinic is open to the public at our office at 6000 Aurora Ave Suite B in Des Moines.
