CSA Hosts Flu Shot Clinic October 5th
Join Community Support Advocates on October 5th to get protected from the flu this year. We will be teaming up with the Polk County Health Department to host a Flu Shot Clinic at our Office on October 5, 2017 from 12-2pm. The Clinic is open the public. All insurances accepted. Please bring a copy of your insurance card. If no insurance bring cash to cover a small fee. A light lunch will be provided for all participants. Call us with any questions! 515-883-1776
