Responsibilities:

The Youth Services Coordinator provides support and skill building services to youth ages 13 through 18, and their families. Services are designed to address the behavioral health needs of youth to maximize effective functioning within family, peers, and community in an age-appropriate manner assist youth in remaining within their homes, schools and communities. Services include behavior intervention, crisis intervention, family training, skills training and development, and program development and administration. The primary job duties are to assist in identifying a person’s skills, needs and interests, providing input into the design and implementation of the treatment plan, providing services to youth and families. This position will also collaborate with all programs at CSA to provide clinical support. The candidate must be well organized, ability to engage youth and family members, excellent verbal and written skills and have a general working knowledge of using an electronic medical
record for treatment planning and progress notes. The YSC providers work with youth and their families on a weekly basis in the family home, community or office. The candidate must work a flexible schedule that may include day, evening and weekend shifts and hours to meet program needs and travel as necessary to provide service.

Qualifications:

Must have a Bachelor’s degree in social sciences plus 1 year of experience as Behavioral Health Intervention Specialist or 20 hours in child mental health, or a Bachelor’s degree in a non-social science field plus 2 years of experience as Behavioral Health Specialist, or 30 hours training in child mental health.

Starting Pay: $17.85/hr

Department: Service Coordination

Responsibilities:

Provides care coordination for members; coordinates the intake and eligibility process; assists persons in obtaining needed services by making referrals and authorizing regional funding or looking into community supports; monitors service provision; maintains records, systems and data, facilitates or assists in the development of the care plan; serves as a member of an interdisciplinary team working on behalf of the member; completes social histories, assessments and all annual paperwork OR secures these documents from in-house MCO case managers; assists in county-wide shelter/other crisis situations as requested. Also works on the Resource & Referral line at least one three and a half hour shift per week.

Qualifications:

Bachelor’s Degree from an accredited college or university with at least 30 semester hours or its equivalent in the behavioral sciences, education, health care, human service administration, or the social sciences field. Applicants must have at least one year minimum experience in the delivery of human services, specific to the needs and abilities of adults with ID/DD and MI. Strong working knowledge of Polk county regional services and supports is preferred. Person must be very flexible, organized and willing to work collaboratively in a team based environment. Must possess a valid driver’s license with a good driving record and have reliable transportation.

Starting Pay: $19.23/hr
